I need help with a stalker....

I have this ex-boyfirend of mine who doesn't take no for an answer. He has threaten me and my boyfirends with death threats. He terrifieds me so much that I can't sleep or I have night mares about him killing me or my boyfirend. My Ex-boyfirend's name is Dustin. Dustin Cooper, he scares me so much. I cant seem to get reid of him now matter what i do. I've had to change my e-mail account,cell phone numbers, online accounts, everything just for him to leave me alone. Well someone plz help me....
